Practitioners or their patients may prepay US BioTek Laboratories directly with check or credit card. Full payment must be enclosed with the requisition form.
We accept VISA, MasterCard, American Express and Discover card.
Bill Account
Practitioners may choose to establish a professional account with a credit card number on file, or apply for payment terms. US BioTek Laboratories reserves the right to assign credit limits, and hold testing for clients unable to maintain a good payment status.
Insurance
US BioTek Laboratories does not bill private insurance for test fees and will not file a claim on the patient's behalf. Patients may request an itemized receipt that they may then submit to their insurance provider. Coverage under an insurance plan may be subject to limits set by the insurance provider.
Billing for Specimen Rejection
Practitioners will be assessed a fee of $15.00 for specimens that are rejected due to improper collection, insufficient quantity or unlabeled samples.
Cancellation or Refund
Tests cannot be cancelled and are non-refundable once the specimens are received by the lab.
Draw Station Fees
US BioTek Laboratories is not contracted with any blood draw stations. Practitioner and/ or patient is responsible for all draw station fees for service.
Medicare
US Biotek Laboratories is not a Medicare or Medicaid provider, and our tests are not reimbursable by either agency.
